Single Payment Whole Life

Plan Description: Single payment of premium required. Death benefit payable at Insured’s death prior to maturity date (policy anniversary following 100th birthday). Net cash value payable on maturity date if Insured is then living.

Separate schedules are provided for nonsmokers (both males and females) and smokers (both males and females). To qualify for nonsmoker rates the applicant may not have used tobacco products in the past 12 months.

Maximum in Missouri and Kansas - $25,000; maximum in Iowa - $10,000
Policy Fee: $100. Fee is the same, regardless of face amount or issue age.
Application: The application to be used is Form 106-80A 020806. Conditional Coverage Receipt Form CCR 02-06 is to be given for cash received with the application.
Substandard: This plan will not be issued at sub-standard rates. Applicant will either be accepted or rejected at standard rates.
Riders: No supplementary benefit riders may be attached to this plan.
Withdrawals: Partial withdrawals or surrenders of cash values are not permitted; however, policy loans are available.
21 Day Refund: Interest sensitive life products contain a 21- Day Right to Examine Policy and to request a full refund of premium. This differs from the period used in other policies.
Non-Medical Limits:

Non-medical limits for the Interest Sensitive Single Payment Whole Life Policy are $6,000 for issue ages 45-60 and $5,000 for issue ages 61-80. However, this will not preclude the company from obtaining medical records or attending physicians statements or from requiring a medical examination in any individual case, where deemed necessary to properly evaluate the risk.

Plan Description: Face amount payable upon death at any time. Premiums payable for 20 years or until prior death.
Issue Age: 0-80
Minimum Amount:

Maximum Amount: $9,900. ($4,900 for issue ages 0-20)
Premiums: Monthly bank draft premiums per $1,000. Premiums for larger amounts are directly proportionate. Premiums for modes other than monthly are multiples of monthly premium. For direct billing, calculate the bank draft premium for appropriate mode; then add a charge of .50 cents.

Plan Description: Face amount payable upon death at any time. Premiums payable for 10 years or until prior death.

Single Premium or Flexible Premium Deferred Annuity

Plan Description: The death benefit is payable at annuitant’s death prior to maturity date (policy anniversary on or before age 110). Income is payable on the maturity date. The Annuity Fund Value is payable at any time prior to maturity date.
Current Interest Rates: The interest rate currently credited to the Annuity Fund Value is 5%. The company, at its option, may adjust the current interest rate paid on all contracts regardless of their age.
Rates: No minimum amount required with application. Premium payments are random.
Issue Age: 0-99 (age nearest birthday)
Application: Form 109-80A DA Rev. 6-13-05. There are no health questions or substandard ratings.
Riders: No Supplementary Benefit Riders may be attached to this plan.
Partial Surrenders: Partial withdrawals or surrenders of the Annuity Fund Value are permitted. Policy loans are not permitted.
20 Day Refund:

Life Paid-Up At Age 95

Plan Description:

Face amount payable upon death at any time. Premiums payable to age 95 or until prior death.

Smoker/Non-Smoker:

Separate rate schedules are provided for smokers and non-smokers. To qualify for non-smoker rates the applicant may not have used tobacco products in the past 12 months.
Issue Ages: 20-80 (age last birthday)
Minimum Amount: $5,000

Discount for Size and direct billing:

The rate schedules show monthly bank draft premiums per "$5,000 Policy" (minimum amount) for both smokers and non-smokers. Premiums are also shown for "each additional $1,000", at a reduced rate per $1,000. Where direct billing is requested, each payment will be increased by 50 cents.
Application: Application Form 80-APP Rev. 03-03 (or any subsequent replacement thereof) must be used. Agents must also give the customary Conditional Coverage Receipt (Form CCR-80) for premiums collected. However, for amounts exceeding $100,000 (including existing coverage with CRI), no money should be collected.
Available Riders:

Waiver of Premium and Guaranteed Purchase Option riders are not available with the LP95 plan since they would require a dual rate structure for Smokers/Non-Smokers. Double Indemnity Accidental Death riders can be added at the following rates per $1,000:

Ages 20-40: Annual $2.00; SA $1.02; Qtly $.51; Monthly $.17
Ages 41-55: Annual $2.50; SA $1.28; Qtly $.64; Monthly $.22

Plan Description:

Face amount payable upon death at any time. Premiums payable to age 65 or until prior death.

Issue Ages: 0-50 (age last birthday)
Minimum Amount: $2,500

Application:

Application Form 80-APP Rev. 03-03 (or any subsequent replacement thereof) must be used. Agents must also give the customary Conditional Coverage Receipt (Form CCR-80) for premiums collected. However, for amounts exceeding $100,000 (including existing coverage with CRL), no money should be collected.
Available Riders: Waiver of Premium, Double Indemnity, Guaranteed Purchase Option, and Home Protector riders may be included in CV-100-PLUS.
Special Features:

Cash values are never less than the total of "standard" policy premiums paid (excluding any "substandard" premiums and any premiums paid for supplementary benefit riders attached to the policy).
